Function First Therapy provides cash-based OT services in your home and community. At this time, we do not accept traditional insurance because we want to focus all our time providing quality care to our main priority... YOU! Upon request, we provide a "superbill" which lists diagnosis and CPT codes that the client can submit to their insurance for reimbursement. Process to start OT services
1. Call for your free phone consultation
This allows your therapist to understand your needs and see if occupational therapy is right for you. If you would benefit from another type of therapy, we will refer you!
2. In-person evaluation
The therapist will come to your home and meet with you for one hour to perform assessments and collaborate on your plan of care.
3. In-person treatments & telehealth consults
You and your therapist will decide what is the best way for you to meet your goals-- In-person treatments or telehealth consults.
Pricing Info
Initial Evaluation: $150
One hour in-person evaluation of your home and unique needs. The therapist will then create your plan of care and provide recommendations to help you meet your goals
Treatments: $100
One hour, hands-on treatment to help you meet your goals. Can be performed in your home or any place in the community that is important to you
Telehealth consult: $75
Therapist provides a 45 minute consultation via telehealth on health or adaptive equipment/ home modification questions
